Incident declaration

All Safety Incidents occurring on the CERN site or related to CERN activities must be declared. This includes commuting accidents.

The responsibilities in matters of safety incident management at CERN are specified in the Safety Regulation SR-SIM available on https://hse.cern/rules.

The procedures related to the declaration of Safety Incidents are detailed in the admin e-guide.

As per SR-SIM, the term Safety Incident covers Radiological Events, Deviations, Unsafe Situations, Near Misses and Accidents, including Commuting Accidents, all of which are defined below:

  • Accident: an unintended event resulting in injuries to persons or in material damage or having a negative impact on the environment.
  • Deviation: a deviation from the normal regulatory functioning of an installation or activity whose consequences or potential consequences are non-negligible from the point of view of safety.
  • Near miss: an unintended event that could have led to an accident.
  • Radiological event: a deviation from the applicable Radiation Protection or Radiation Safety rules.
  • Road-related Safety Incident: Safety Incidents occurring on a CERN road or car park or involving any type of vehicle used for commuting or in relation with CERN activities.
  • Unsafe situation: a situation that may lead to a radiological event, a near miss or an accident.

All Safety Incidents shall be declared in EDH within 24h to ensure a proper follow-up: https://edh.cern.ch/Document/General/IncidentDeclaration.

In case of injury, also submit a Declaration of occupational accident (HS50) in EDH within 10 working days of the accident to allow the classification by HR, https://edh.cern.ch/Document/General/HS50, and attach a medical certificate detailing injury. According to the procedure, https://admin-eguide.web.cern.ch/en/professional-accident-and-medical-certificate, the medical certificate must state that the leave is due to an occupational accident and state the date. This information can only be provided by the doctor.
